Removal of suspicious skin lesions

Newly appearing or changing skin lesions should always be examined by a qualified specialist. Once a lesion has been removed, it is routinely submitted for histopathological examination.
Complete removal with histological confirmation of tumor-free margins is the gold standard of treatment. Once skin cancer has been fully excised, reconstruction of the resulting tissue defect can be performed. Depending on the facial region involved, different reconstructive techniques may be required.

Nasal reconstruction

Reconstruction of nasal defects can be particularly challenging. I have been working in the field of nasal reconstruction for over 20 years and was the first to describe the technique of prelamination in nasal reconstruction. This method allows especially effective and tissue-sparing restoration of the nasal lining and has been adopted by several international centers, including Johns Hopkins Medical Center in the United States.
A distinction is made between defects limited to the external skin and more complex defects that require replacement of the structural framework of the nose—bone and cartilage—and, in some cases, the internal mucosal lining.
Depending on the complexity of the defect, reconstruction may require multiple surgical stages spaced several weeks apart. For example, nasal dorsal skin may be reconstructed using a forehead flap, while structural elements of the nasal skeleton may be restored using cartilage harvested from the ear or rib.
Nasal reconstruction may involve one or several procedures. If complete tumor removal—such as after excision of a basal cell carcinoma—has been confirmed, the first reconstructive step can usually be planned approximately one week after tumor removal. Complete oncologic clearance is always the essential prerequisite.
I perform a large number of nasal reconstructions, including during my regular humanitarian surgical missions in Africa.

Eyelid reconstruction

Correction of eyelid defects may require complex surgical techniques. In some cases, multiple stages are necessary—for example, when tissue from the upper eyelid is transferred to reconstruct the lower eyelid.
Additional important techniques include reconstruction of the eyelid margin and the lacrimal drainage system. I perform a high volume of these procedures, including during my regular humanitarian missions in Africa.

Ear reconstruction

Reconstruction of the external ear ranges from repair of small defects to complete auricular reconstruction. All components of the ear—including the earlobe, ear canal, and auricle—may be affected, and specific reconstructive techniques exist for each region.
Replacement of structural elements may require harvesting and transplantation of auricular or rib cartilage. In many cases, procedures can be performed on an outpatient basis under local anesthesia. More complex reconstructions may require staged procedures and, in some cases, general anesthesia.
